LAUTECH Medical Registration Exercise for New Students, 2022/2023 Academic Session

- A copy of your Personal Biodata form
- Duly filled Medical Registration form
- Copies of Medical and Tuition fee receipt or printout
- X-ray film
- Medical Laboratory results
- Two (2) passport photographs
Step 4: Obtain LAUTECH certificate of medical clearance.
The record officer will provide you with the medical confidential file to complete, compile your supporting documentation, and then direct you to the medical officer for the student medical clearance certificate issuance and final clinical assessment required for your faculty registration.
Step 5: Keep Your Health Center Card
Keep your health center card because it is your method of identification for obtaining medical services anytime you visit the facility. It was handed to you by the record officer.
